📌 Key Responsibilities

• Maintain, organize, and retrieve patient medical records in physical and electronic formats per NABH standards
• Code patient diagnoses and procedures using ICD-10 and CPT coding systems for accurate health statistics
• Ensure completeness of medical records including discharge summaries, OT notes, and consent forms
• Coordinate with clinical departments for pending documentation and record completion within defined timelines
• Process requests for medical records, certificates, and summaries from patients, insurance companies, and courts
• Maintain death and birth registers, MLC records, and medico-legal documentation as per hospital policy
• Prepare monthly health statistics, patient census, and disease-wise morbidity reports

🎓 Qualifications and Requirements

• B.Sc. Medical Records Science or DHMRM (Diploma in Health and Medical Records Management) preferred
• Any graduate with hospital medical records training certificate also considered
• 1 - 4 Years of medical records department experience in a NABH-accredited hospital
• Knowledge of ICD-10 coding, CPT coding, and medical terminology
• Familiarity with Electronic Medical Records (EMR) systems and HMS
• Good documentation, confidentiality, and attention to detail in medical record handling
• Understanding of NABH standards and medico-legal documentation requirements
• Good Tamil and English communication skills for team coordination and client interaction in Coimbatore.
