• Communicate visit timelines, care updates, and staff assignments to patient families proactively
• Track care plan adherence and follow up with clinical staff on pending documentation and visit notes
• Handle patient and family complaints, service escalations, and re-scheduling requests promptly
• Coordinate with the nursing supervisor and operations team for staff gaps or emergency visit coverage
• Maintain patient records, visit logs, and billing data in the home care management software
• Generate daily and weekly MIS reports on visit completion, patient status, and staff attendance
• Support onboarding of new patients by explaining the care process, schedule, and emergency contacts

🎓 Qualifications and Requirements

• Do you have experience in patient scheduling or service coordination in a healthcare setting?
• Are you comfortable managing multiple phone calls, messages, and escalations simultaneously?
• Can you operate home care software or any patient scheduling and tracking system?
• Have you coordinated between clinical staff, patients, and families in a time-sensitive environment?
• Are you familiar with routing and zone-based staff allocation for field teams in Chennai?
• Can you handle patient family complaints calmly and resolve escalations efficiently?
• Are you comfortable generating MIS reports and maintaining accurate digital records?
• Good Tamil and English communication skills for team coordination and client interaction in Chennai.
