• Administer advanced IV therapy, infusions, and complex medication regimens at patient homes
• Manage wound care, stoma, tracheostomy, and catheter care for high-dependency home patients
• Counsel patients and families on disease management, dietary requirements, and lifestyle modifications
• Coordinate care across physiotherapists, attendants, and the treating physician for each patient
• Document all clinical findings in the company's electronic health records or care app
• Monitor and respond to changes in patient condition and escalate to doctors when needed
• Mentor junior nurses and attendants on clinical protocols during shared home visits

🎓 Qualifications and Requirements

• Do you hold a B.Sc. Nursing degree with valid state nursing council registration?
• Have you managed complex home care patients including tracheostomy or infusion therapy?
• Can you independently develop and implement nursing care plans for diverse patient categories?
• Are you proficient in using electronic health records or digital care management apps?
• Do you have experience coordinating care with physiotherapists and attendants in a home setting?
• Are you comfortable covering multiple patient zones across Chennai in a single shift?
