📌 Key Responsibilities

• Assess wound type, stage, and healing progress at each home visit and document findings with photographs
• Select and apply appropriate dressing materials including hydrocolloid, foam, alginate, and antimicrobial dressings
• Manage stoma care including colostomy, ileostomy, and urostomy bags and skin barrier maintenance
• Educate patients and families on wound hygiene, dressing change techniques, and infection warning signs
• Monitor diabetic foot ulcers, pressure injuries, and post-surgical wounds for signs of infection or dehiscence
• Coordinate with the treating surgeon or physician for wound treatment plan updates
• Maintain wound care supply kit and track consumable usage per patient visit
• Document wound healing progress in the clinical records system and report non-healing wounds immediately

🎓 Qualifications and Requirements

• Do you have hands-on experience in wound assessment and advanced wound dressing techniques?
• Have you managed stoma care including colostomy and ileostomy in a clinical or home setting?
• Are you trained in diabetic foot ulcer management and pressure injury prevention?
• Can you photograph and document wound progress accurately at each home visit?
• Do you have experience educating patient families on wound hygiene and self-care?
• Are you comfortable travelling across Chennai zones for multiple wound care home visits per day?
• Have you coordinated with surgeons or physicians on wound management plans?
• Good Tamil and English communication skills for team coordination and client interaction in Chennai.

GNM or B.Sc. Nursing with state nursing council registration for home nursing roles; BPT for home physiotherapy roles; D.Pharm for medication management roles; Basic Caregiver Certification (NSDC) for patient attendant roles; BLS and First Aid certification for all clinical home care roles; palliative care certification for oncology or elderly home care; wound care and catheter management training; dementia care and elderly handling certification
