• Conduct physiotherapy assessments and treatment sessions at patients' homes using portable equipment
• Deliver post-surgical, neurological, and geriatric rehabilitation programmes in a home setting
• Educate family members and caregivers on safe patient handling, positioning, and home exercise routines
• Maintain detailed session notes and progress records and share reports with referring clinicians
• Manage a daily schedule of home visits across assigned zones with punctuality and efficiency
• Identify safety hazards in the home environment and advise on adaptive equipment and modifications
• Coordinate with the clinic coordinator for appointment scheduling, route planning, and urgent case escalation
