📌 Key Responsibilities

• Coordinate and schedule rehabilitation appointments for patients across physiotherapy, occupational therapy, and speech therapy disciplines within the clinic
• Track patient attendance, session completion, and progress against rehabilitation goals and report to treating therapists and clinic management
• Act as the primary point of contact for patients and families regarding rehabilitation programme timelines, appointments, and queries
• Facilitate multidisciplinary team meetings and document care plans, review notes, and discharge summaries
• Manage insurance pre-authorisation requests, liaise with TPA teams, and follow up on pending approvals for rehabilitation packages
• Coordinate discharge planning including home exercise programme handover, caregiver training scheduling, and follow-up appointment booking
• Maintain accurate patient records in the clinic EMR system and prepare monthly reports on patient throughput and programme completion rates

• How do you coordinate a patient's rehabilitation schedule across physiotherapy, OT, and speech therapy?
• Describe how you track a patient's progress through a multi-week rehabilitation programme.
